Commissioner of Oaths

Legislative and Information Services at Town Hall provides a commissioner of oaths service for $15. Please check that your document allows signing by a commissioner of oaths.

Birth Registration/Birth Certificates

All births occurring at the Rouge Valley Ajax-Pickering Health Centre and homebirths occurring in Ajax must be registered through Legislative and Information Services.

  • Birth registration takes approximately 3 months; the fee for registering a birth is $25.
  • If you require documentation to enable you to travel with your new baby, a travel letter may be issued from this office; the fee is $25.

To complete your newborn's birth registration form completely and accurately, visit ServiceOntario.  With a few additional clicks, complete an online application for a birth certificate and Social Insurance Number.  Once you have completed the Statement of Live Birth, print and ensure that the form is appropriately signed.  Please forward by mail or in person to:  Town of Ajax, Legislative & Information Services Department, 65 Harwood Avenue, S., Ajax, Ontario L1S 2H9.  A fee of $25.00 is payable to the Town of Ajax, by cash, cheque, money order or debit.

If you do not have access to the internet, complete the Statement of Live Birth form in the package supplied by the hospital.  Once completed, forward by mail or in person to:  Town of Ajax, Legislative & Information Services Department, 65 Harwood Avenue, S., Ajax, Ontario L1S 2H9.  A fee of $25.00 is payable to the Town of Ajax, by cash, cheque, money order or debit.

Marriage Licences

Marriage Licence Applications can be processed in the Legislative and Information Services Department. Our fee for the licence is $100. It takes about 15 minutes to process the form. We're open weekdays from 8:30 a.m. - 4:30 p.m. 

Marriage licence requirements:

  • You must completely fill out the application and both the applicant and joint applicant must sign it. Only one person needs to come into the office as long as they provide proof of age of their fiancé (birth certificate or passport).
  • Those under the age of 18 require both parents' consent.
  • A marriage licence can only be issued to the applicant and/or joint applicant.
  • If you have been married before, we require: The original or court certified copy of the Decree Absolute or Certificate of Divorce. It cannot be a photocopy; you can get a certified copy from the court that granted the divorce. If the divorce was granted outside Canada, please contact the Legislative and Information Services Department for further information at 905-619-2529, ext. 338.
  • The fee ($100) is payable by cash or interac and the licence is valid for three months anywhere in Ontario.
  • You do not need to have any blood tests.
  • You do not have to live in Ajax to get a marriage licence through the Ajax office.

Civil Marriage Ceremonies are now being offered by the Town of Ajax.

Lottery Licences

The Town of Ajax issues lottery licences in accordance with Provincial regulations for Bingo Lotteries up to a $5,500.00 prize board, Raffle Lotteries with a Prize board of $50,000.00 and under, and Break Open Tickets. Please visit Lottery Licences for further information.

Records Management & FOI Requests

Requests For Information under the Freedom of Information Act must be made in writing to the Records Co-ordinator and must include a $5 fee. Search time, photocopying and electronic rendering of requested material may be subject to further fees.
