- Copy of Financial Statement for the most recent Fiscal Year of your organization.

- Current listing of Board of Directors, including addresses and phone numbers, dated and signed.
A detailed outline of the programs/services provided.
- Details outlining the proposed use of lottery proceeds.
- Details outlining that the organization is established to provide charitable services in Ajax/Ontario and that the proceeds will be used for services that will benefit Ajax residents.
- A lottery trust account with two signing officers of the charity who have cheque writing privileges. All expenses and charitable donations must be paid by cheque and shall be directly related to the conduct of the event.
